Alberta Unveils New Payment Model for Family Doctors

After months of deliberation, Alberta’s UCP government has unveiled a new pay model for family doctors, aiming to improve healthcare access and retain physicians within the province. Premier Danielle Smith and Health Minister Adriana LaGrange announced the new initiative during an afternoon press conference on Thursday.

A Strategic Focus on Physician Retention and Recruitment

The Alberta government asserts that this new compensation framework will attract more doctors to the province while encouraging existing physicians to stay. The model focuses on three key billing components:

  • Patient interactions
  • Panel size (requiring a minimum of 500 patients)
  • Time spent on direct or indirect care

Co-developed with the Alberta Medical Association (AMA), the new structure aims to address the financial challenges that family doctors have faced in recent years.

“We have worked with the AMA to address the challenges that primary care physicians are facing,” said Health Minister LaGrange in a statement. “This model will provide the supports physicians need and improve patient access to the care they need.”

Major Investments to Bolster Healthcare Services

The pay model is part of a broader strategy to strengthen Alberta’s healthcare system. Key highlights include:

  • A $42 million investment to recruit additional healthcare providers and expand essential services.
  • A rural and remote bursary program for family medicine resident doctors.
  • An additional $257 million in funding to stabilize primary care delivery and improve access to family physicians.

The province emphasizes that the new structure incentivizes:

  • High patient numbers.
  • Providing after-hours care.
  • Enhancing team-based care.

Implementation Timeline

Enrolment for the new pay model will begin in January 2025, with full implementation expected by spring 2025. The government has stated that at least 500 doctors need to enroll for the plan to move forward.

Addressing Physician Concerns

The announcement comes after an AMA survey earlier this year revealed significant concerns among Alberta’s family doctors:

  • Over 90% expressed worries about the financial viability of their practices.
  • More than 60% considered leaving the province due to these challenges.

The AMA has noted that delays in reaching an agreement have negatively impacted the healthcare system and patients’ access to family physicians.

“Family medicine is the foundation of our healthcare system,” said AMA President Shelley Duggan. “This model recognizes the extensive training, experience, and leadership of primary care physicians, and we hope it will help Alberta to attract and retain more family medicine specialists who provide comprehensive care.”

Mixed Reactions to the New Model

While the new pay model has received praise for its potential benefits, it has also faced criticism for delays. Opposition NDP health critic Sarah Hoffman called it a “good first step” but noted that the agreement should have been finalized months ago.

“Although it is better late than never, this agreement should have been done back in May when the premier promised it would be signed within a couple of weeks,” Hoffman stated. “Hopefully, this will stop the further hemorrhaging of healthcare workers who have had to close practices, move away from our province, and even leave the profession.”
